Pregnancy outcomes in women with type 1 diabetes in China during 2004 to 2014: A retrospective study (the CARNATION Study)

Highlights
- This retrospective study is the first to report pregnancy outcomes among Chinese women with type 1 diabetes during 2004 to 2014.
- We found that the pregnancy outcomes were poor among this population, despite dramatic improvement in the outcomes of the maternal care system for the general population, scientific progress, and release of guidelines.
- Our findings implicate a great burden of women with type 1 diabetes and that there is an urgent need to take measures to reduce adverse pregnancy outcomes.

- Approximately 63.6% of diabetes patients with an estimated glomerular filtration rate (eGFR) < 60 mL/min/1.73 m2 have normal albuminuria.
- The factors related to normoalbuminuria and eGFR < 60 mL/min/1.73 m2 were male sex, older age, higher triglyceride levels, lower extremity arterial disease, and a longer duration of diabetes.
- Compared with patients with both albuminuria and eGFR decline, those with normoalbuminuria have better control of metabolic indicators.

- Meta-analysis of α- and β-cell transcriptomes reveals modified energy metabolism, immune response, autophagy, and cellular stress pathways in cells from type 2 diabetes mellitus.
- The proportion of immature and senescent β-cells is increased in islets from type 2 diabetes mellitus.
- α- and β-cells from type 2 diabetes mellitus have an increased nuclear factor erythroid 2-related factor 2 (NFE2L2)-mediated antioxidant response.
- Immature and senescent β-cells had increased NFE2L2-mediated antioxidant response, and this suggests that NFE2L2 plays a role in β-cell dysfunction and maturation.

- The proportions of individuals with obesity, abdominal obesity, and heavy drinking increased according to fasting glucose level.
- The risk for uncontrolled blood pressure and lipid profiles increased with increasing fasting glucose levels.
- The increases in risks for uncontrolled blood pressure and lipid profiles were mostly observed for fasting glucose levels ≥90 mg/dl.
- Efforts are needed to prevent increased fasting glucose levels, as higher levels, even within normal range, were associated with poor metabolic profiles.
