Inter-Practice Variability in General Practice Consultations With Older Patients: A Cross-Sectional Analysis of the Registrar Clinical Encounters in Training Study

Summary

Managing older patients (those aged 65 and above) is crucial for GP trainees' education and training in management of chronic diseases. It is known, however, that on average, GP trainees see fewer older patients than established GPs. This study aimed to assess the difference in number of older patients seen by trainees, based upon the practice in which they are training. A cross-sectional analysis of trainee consultations from 2010 to 2023 was performed. 19% of trainee consultations were with an older patient. By changing training practice location, the median odds of a trainee consulting with an older patient approximately doubles or, alternatively, halves (meaning that there will be two-fold, or half, the likelihood of a registrar seeing older patients with a change of practice). Controlling for other potential confounding factors, like registrar and patient variables, found little difference in median odds, and ‘structural’ practice factors (e.g. size of practice and location) had only minor effects. We conclude that GP clinic-level interventions are essential to improve GP trainee engagement with older patients.
