Pneumonia with pleuritis is a rare presentation of histoplasmosis infection. We present a 12-year old previously healthy boy in whom histoplasmosis presented with pleuritis, confirmed by detection of antigen in empyema fluid. Pediatr. Pulmonol. 2011; 46:729–731. © 2011 Wiley-Liss, Inc.
