Summary

The management of peptic ulcer disease (PUD) and the realisation of the role of Helicobacter pylori in the pathogenesis of PUD has seen a marked reduction in the incidence of complicated PUD and its need for surgery. This chapter summarises the aetiology of PUD in the light of these changes, the clinical diagnosis of the disease and the management of its complications. Modern laparoscopic surgery, embolisation for acute bleeding, endoscopic control of bleeding and traditional/historical surgery are explained to allow a greater understanding of the management of the disease. Cardinal clinical features are outlined and investigations for refractory disease explained.
