The subject of this report was discussed at a special session of the Annual Meeting of the Peripheral Nerve Society in St. Paul, Minnesota, on June 16, 1994. Attendees were invited to participate in modifying the report and in naming others who should be asked to participate. The report was sent to contributors (up to six times), who modified it and signed that they had read and agreed with the final report. See Appendix for complete list of contributors.
