Single tooth restoration with pin-guide system: A single case presentation

Background: Case report: This case report describes the use of a single implant restoration using a pin-guide navigation system (IBS MagicPin-Guide) to position a one-phase implant (IBS MagiCore) optimally in compromised available bone, showing that a simple guide system can be an alternative to pre-fabricated guides.
Materials and Methods: A middle aged healthy patient presented to our clinic with a missing upper right second premolar. The tooth was removed approximately a year before, due to unsuccessful endodontic treatment. Available bone was compromised. Patient had wide bone, but bone height was compromised. Patient either needing a sinus augmentation or careful positioning of the implant into a bone-ridge was needed. A single tooth restoration was planned on a one-phase implant (IBS MagiCore). After inserting a pin-guide an intraoperative CB-CT scan was done, the pin was checked, and an 4.0/11 mm IBS MagiCore implant was placed transgingivally. Burring was done with a hollow drill, over the inserted pin. No flap was raised.
Results: Postoperative healing was uneventful. After a healing period of 4 months, a screw retained porcelain fused metal crown was fabricated. The implant was followed up for 6 months. There was no bone resorption, gum recession. The patient was satisfied with the result. The preoperative plan was met, the available bone was used optimally.
Conclusions and Clinical Implications: Placement of dental implants have to be driven by prosthetic considerations. Single implant placement may be challenging due to the prosthetic situation and available bone. Positioning of one-phase implants are even more challenging because of the possible corrections in angulation. There are several guide systems on the market. In this poster we introduced our experiences with a single one-phase implant restoration using a guide-pin system. The IBS method is straightforward, fast and easy to use.
