The limitations of preoperative grading of rectal carcinoma

Abstract

In 103 patients who had a preoperative biopsy before resection of a single carcinoma of the rectum, the grading of the biopsy by one pathologist was compared to the definitive grade. Only 62% of the biopsies were correctly assessed, and the tendency was to underestimate the grade with few tumors being described correctly as high grade. This suggests that the grading of biopsy fragments has definite limitations and is of limited value when selecting patients for conservative treatment of rectal carcinoma.
