Abstract

Case management is an approach to social service delivery that attempts to ensure that clients with multiple, complex problems and disabilities receive the services they need in a timely, appropriate fashion. It is practiced in such fields as mental health, child welfare, aging and long-term care, alcohol and drug treatment, health care, and the public welfare system. This article reviews case management's history, current models, and effectiveness with regard to client and service system outcomes. Implications for case management service delivery at the micro, mezzo, and macro levels of social work practice are presented.
